Adding IP Address - Newbie Question

Our host just assigned us an additional IP address for a new store that will use an SSL certificate. However, when I look at the Add IP Address screen, I'm unsure what to enter:

Obviously, I know the IP address but the rest.....

Thanks in advance.

Addendum:

If anyone has a specific solution, I'm willing to pay via PayPal. I'm stuck without this information.
 
If you're willing to pay then my recommendation would be to get an email support contract from sw-soft. I have one and it's well worth the money just for the peace of mind.

The subnet mask will probably be the same as the network adapter's. Log into your server with Remote Desktop and have a look at the network connection settings.

The IP address is exclusive if it's being used by that site only.

You'll also need to install the SSL certificate so that it appears in the certificate drop-down.
