strange changes in .htaccess

Some of the .htaccess files in several domains on my server are changed.
the lines:
AddType application/x-httpd-php .htm .html
php_value auto_append_file "/var/www/vhosts/xxx.com/httpdocs/BBClone/count.php

the count.php is adding two lines to each .php or .html file

how to stop this?
