Hotfix1 for 4.5 version does not update database and Sitebuilder version still stays 4.5.0. You can check if patch installed or not in Server -> Autoupdate section.
As for patch sitebuilder_4.5_design_patch.tgz it does not included into Hotfix1 for 4.5.0 version and should be installed separately. This patch will be included into next update.
